Description
MRPS12 encodes a nuclear-encoded mitochondrial ribosomal protein that is a component of the small (28S) subunit of the mitochondrial ribosome. This protein is essential for mitochondrial translation, enabling the synthesis of 13 subunits of the oxidative phosphorylation (OXPHOS) system encoded by mitochondrial DNA. MRPS12 is synthesized in the cytosol and imported into the mitochondria, where it assembles into the mitoribosome. Defects in MRPS12 can impair mitochondrial protein synthesis, leading to combined OXPHOS deficiencies and mitochondrial disease.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Combined Oxidative Phosphorylation Deficiency 45 (COXPD45) | Loss-of-function mutations in MRPS12 impair mitochondrial translation, reducing the synthesis of OXPHOS subunits and causing energy deficiency. | ClinVar, OMIM |
| Mitochondrial Encephalopathy | Disrupted mitochondrial protein synthesis leads to neurological symptoms, including developmental delay, seizures, and hypotonia. | ClinVar, OMIM |
| Leigh Syndrome Spectrum | Severe OXPHOS defects due to MRPS12 mutations can present with bilateral brainstem lesions and lactic acidosis. | ClinVar, OMIM |

Mutation functional classification
Loss of Function (LOF)
Most reported MRPS12 mutations are loss-of-function, leading to reduced mitochondrial translation and OXPHOS deficiency.
Gain of Function (GOF)
No gain-of-function mutations have been reported for MRPS12.
Dominant Negative (DN)
No dominant-negative mutations have been described; MRPS12 mutations are typically recessive.

Protein Summary
MRPS12 is a 138-amino-acid protein (15.5 kDa) that localizes to the mitochondrial matrix. It is a core component of the 28S small mitoribosomal subunit, where it interacts with 12S rRNA and other ribosomal proteins to form the decoding center. The protein contains a conserved ribosomal S12 domain and is essential for accurate initiation and elongation of mitochondrial translation. MRPS12 is ubiquitously expressed, with highest levels in tissues with high energy demand such as heart and skeletal muscle.
