Description
ICAM1 (Intercellular Adhesion Molecule 1) is a cell surface glycoprotein that mediates adhesion between leukocytes and endothelial cells, playing a critical role in immune surveillance, inflammation, and transendothelial migration. It is a member of the immunoglobulin superfamily and binds to integrins such as LFA-1 (CD11a/CD18) and Mac-1 (CD11b/CD18). ICAM1 expression is upregulated by pro-inflammatory cytokines (e.g., TNF-alpha, IL-1) and is implicated in various inflammatory and autoimmune diseases, as well as cancer metastasis.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Rheumatoid Arthritis | ICAM1 facilitates leukocyte adhesion to synovial endothelium, promoting joint inflammation. | ClinVar, OMIM |
| Atherosclerosis | Upregulated ICAM1 on endothelial cells recruits monocytes, contributing to plaque formation. | NCBI Gene, OMIM |
| Asthma | ICAM1 mediates eosinophil and T-cell adhesion to airway epithelium, exacerbating inflammation. | OMIM, ClinVar |
| Melanoma | ICAM1 expression on tumor cells promotes adhesion to endothelium, aiding metastasis. | COSMIC, NCBI Gene |
| Multiple Sclerosis | ICAM1 is involved in lymphocyte trafficking across the blood-brain barrier. | OMIM, ClinVar |

Mutation functional classification
Loss of Function (LOF)
No well-characterized loss-of-function mutations reported in ICAM1; homozygous knockout in mice is viable but shows impaired immune responses.
Gain of Function (GOF)
Gain-of-function is not established; overexpression in tumors is associated with metastasis.
Dominant Negative (DN)
No dominant-negative mutations described.

Protein Summary
ICAM1 is a 532-amino-acid type I transmembrane glycoprotein with five immunoglobulin-like domains in its extracellular region. It binds integrins LFA-1 and Mac-1, mediating leukocyte adhesion and migration. The protein is heavily glycosylated and expressed on endothelial cells, immune cells, and epithelial cells. Its cytoplasmic tail interacts with the cytoskeleton via alpha-actinin and other adaptors, facilitating signaling. ICAM1 is also a receptor for rhinoviruses and Plasmodium falciparum-infected erythrocytes.
