Description
FOXF1 (Forkhead Box F1) is a transcription factor belonging to the forkhead family, characterized by a conserved DNA-binding forkhead domain. It plays a critical role in embryonic development, particularly in the formation of the lungs, gastrointestinal tract, and vasculature. FOXF1 regulates cell proliferation, differentiation, and migration during organogenesis. Heterozygous deletions or loss-of-function mutations in FOXF1 cause alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV), a severe neonatal lung disorder. The gene is also implicated in vascular development and maintenance of the alveolar epithelium.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) | Heterozygous loss-of-function mutations or deletions in FOXF1 disrupt lung alveolarization and pulmonary vascular development, leading to misalignment of pulmonary veins and capillary hypoplasia. | ClinVar, OMIM #265380 |
| Congenital heart defects | FOXF1 mutations are associated with cardiac anomalies such as ventricular septal defects and aortic arch abnormalities, likely due to its role in mesodermal development. | OMIM, PubMed |
| Gastrointestinal malformations | FOXF1 deficiency impairs gut development, resulting in malrotation, atresia, or other structural anomalies. | OMIM, PubMed |

Mutations & Variants
Hotspot Mutations
| Variant | Type | Frequency | Functional Description |
|---|---|---|---|
| c.250C>T (p.Arg84*) | Nonsense | Rare (ACDMPV) | Loss of function; premature truncation of FOXF1 protein |
| c.307_308delAG (p.Ser103fs) | Frameshift | Rare (ACDMPV) | Loss of function; frameshift leads to truncated protein |
| c.1A>G (p.Met1?) | Start loss | Rare (ACDMPV) | Loss of function; translation initiation abolished |
| Whole gene deletion | Copy number loss | Rare (ACDMPV) | Loss of function; haploinsufficiency |
Mutation functional classification
Loss of Function (LOF)
Most FOXF1 mutations in ACDMPV are loss-of-function (nonsense, frameshift, start loss, deletions) leading to haploinsufficiency. These impair transcriptional activation of target genes essential for lung and vascular development.
Gain of Function (GOF)
No gain-of-function mutations have been reported for FOXF1 in human disease.
Dominant Negative (DN)
Dominant-negative effects are not well documented for FOXF1; the primary mechanism is haploinsufficiency.

Protein Summary
FOXF1 is a 379-amino acid transcription factor containing a highly conserved forkhead DNA-binding domain. It binds to the consensus sequence 5'-RYMAAYA-3' in promoter regions of target genes. The protein is predominantly nuclear and regulates genes involved in cell cycle, extracellular matrix remodeling, and angiogenesis. FOXF1 is essential for alveolar septation and pulmonary vascular formation. In the absence of functional FOXF1, lung capillary alignment fails, leading to lethal respiratory failure in newborns.
