Description

FGF14 (Fibroblast Growth Factor 14) is a member of the fibroblast growth factor (FGF) family, but unlike canonical FGFs, it functions primarily as an intracellular protein (iFGF) that modulates voltage-gated sodium channels (Nav) and other ion channels in neurons. It is highly expressed in the central nervous system, particularly in the cerebellum, and is critical for neuronal excitability, synaptic transmission, and motor coordination. Mutations in FGF14 cause spinocerebellar ataxia 27 (SCA27), a neurodegenerative disorder characterized by progressive ataxia, tremor, and cognitive impairment.

Disease Associations

Disease category Pathophysiological mechanism Genomic evidence
Spinocerebellar ataxia 27 (SCA27) Loss-of-function mutations in FGF14 impair Nav channel modulation, leading to reduced neuronal excitability and cerebellar degeneration. OMIM #609307; ClinVar; multiple case reports
Spinocerebellar ataxia 27B (SCA27B) Intronic GAA repeat expansions in FGF14 cause late-onset cerebellar ataxia with reduced penetrance. PMID: 36599369; PMID: 36600070; ClinVar
Episodic ataxia Missense mutations (e.g., p.Phe145Ser) disrupt FGF14-Nav interaction, leading to episodic motor incoordination. PMID: 12874400; OMIM

Mutations & Variants

Hotspot Mutations
Variant Type Frequency Functional Description
c.434T>C (p.Phe145Ser) Missense Rare Disrupts Nav1.6 binding; causes SCA27
c.430A>G (p.Asn144Asp) Missense Rare Reduced channel modulation; SCA27
GAA repeat expansion (intron 1) Repeat expansion 0.1-0.5% in ataxia cohorts Late-onset SCA27B; reduced FGF14 expression
c.1A>G (p.Met1Val) Start loss Very rare Complete loss of function; severe SCA27
Mutation functional classification

Loss of Function (LOF)

Most SCA27 missense and truncating mutations reduce FGF14's ability to modulate Nav channels, leading to decreased neuronal excitability.

Gain of Function (GOF)

Not established; no activating mutations reported.

Dominant Negative (DN)

Some missense mutants (e.g., p.Phe145Ser) may interfere with wild-type FGF14 function in heterozygous state.

Pathways

• REACT:13685 – Voltage gated sodium channel complex
• WP:WP2848 – Spinocerebellar ataxia pathways
• WP:WP3932 – Fibroblast growth factor signaling

Protein Summary

FGF14 (also known as FHF4) is a 247-amino acid intracellular fibroblast growth factor homologous factor (iFHF) that lacks a classical signal peptide and does not act as a secreted growth factor. Instead, it binds directly to the C-terminal tail of voltage-gated sodium channels (Nav1.1, Nav1.2, Nav1.6) and modulates their gating properties, including inactivation kinetics and resurgent current. FGF14 is essential for normal cerebellar Purkinje cell firing and motor coordination. Loss of FGF14 function leads to impaired sodium channel activity, reduced neuronal excitability, and progressive ataxia.
