Description
CEACAM5 encodes carcinoembryonic antigen (CEA), a glycosylphosphatidylinositol (GPI)-anchored cell surface glycoprotein involved in cell adhesion, intracellular signaling, and immune modulation. It is normally expressed during fetal development and at low levels in adult colonic epithelium, but is frequently overexpressed in colorectal, gastric, pancreatic, lung, and breast cancers. CEA is a widely used serum biomarker for monitoring colorectal cancer recurrence and response to therapy.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Colorectal cancer | Overexpression of CEACAM5 promotes tumor cell adhesion, invasion, and immune evasion via inhibition of NK cell activity and modulation of integrin signaling. | ClinVar, COSMIC, NCBI |
| Pancreatic cancer | Elevated CEA levels correlate with tumor burden and poor prognosis; aberrant glycosylation alters adhesion properties. | NCBI, OMIM |
| Gastric cancer | CEACAM5 overexpression is associated with advanced stage and lymph node metastasis. | NCBI, COSMIC |
| Non-small cell lung cancer | CEA is used as a serum marker; high expression linked to reduced survival. | ClinVar, NCBI |
| Breast cancer | CEACAM5 expression is upregulated in a subset of tumors and may contribute to metastatic spread. | NCBI, COSMIC |

Loss of Function (LOF)
No well-characterized loss-of-function mutations reported in CEACAM5; most variants are rare missense changes with uncertain significance.
Gain of Function (GOF)
Overexpression (not mutation) is the primary gain-of-function mechanism in cancer; no activating mutations have been described.
Dominant Negative (DN)
No dominant-negative mutations have been reported for CEACAM5.

Protein Summary
CEACAM5 (CEA) is a 180 kDa GPI-anchored glycoprotein composed of seven immunoglobulin-like domains (one N-terminal V-set domain and six C2-set domains). It mediates homophilic and heterophilic cell adhesion, modulates integrin signaling, and suppresses immune responses by inhibiting NK cell cytotoxicity and inducing T-cell anergy. In cancer, overexpression of CEACAM5 facilitates tumor cell aggregation, invasion, and immune escape. Its serum level is a standard biomarker for colorectal cancer management.
