Description
CEACAM1 encodes a member of the carcinoembryonic antigen (CEA) family of cell adhesion molecules. The protein is a transmembrane glycoprotein involved in cell-cell adhesion, immune modulation, angiogenesis, and tumor suppression. It is expressed on epithelial cells, endothelial cells, and immune cells, and its expression is frequently altered in various cancers. Alternative splicing generates multiple isoforms with distinct cytoplasmic tails that mediate differential signaling.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Colorectal Cancer | Loss of CEACAM1 expression promotes tumor invasion and metastasis; aberrant splicing contributes to immune evasion. | ClinVar, COSMIC, NCBI |
| Melanoma | CEACAM1 overexpression on tumor cells correlates with poor prognosis and immune checkpoint activity. | ClinVar, NCBI |
| Non-Small Cell Lung Cancer | Reduced CEACAM1 expression is associated with advanced stage and lymph node metastasis. | COSMIC, NCBI |
| Inflammatory Bowel Disease | CEACAM1 polymorphisms and altered expression linked to intestinal inflammation and barrier dysfunction. | OMIM, NCBI |
| Type 2 Diabetes | CEACAM1 variants influence insulin clearance and hepatic lipid metabolism. | OMIM, NCBI |

Mutation functional classification
Loss of Function (LOF)
Missense mutations affecting the Ig-like domains or start codon reduce cell surface expression and adhesion, promoting tumor invasion.
Gain of Function (GOF)
Not well documented; some splice variants with longer cytoplasmic tail may enhance immune inhibitory signaling.
Dominant Negative (DN)
Isoforms lacking the cytoplasmic domain can interfere with full-length CEACAM1 signaling, modulating immune responses.

Protein Summary
CEACAM1 is a transmembrane glycoprotein of the CEA family, characterized by N-terminal Ig-like V-type and C2-type domains. It mediates homophilic and heterophilic cell adhesion, modulates T-cell and NK-cell activity, and regulates insulin clearance. The protein exists as multiple splice isoforms (long and short cytoplasmic tails) that differentially recruit signaling molecules such as SHP-1 and SHP-2. CEACAM1 is implicated in tumor suppression, immune evasion, and metabolic homeostasis.
