Description
The AGT gene encodes angiotensinogen, a precursor protein of the renin-angiotensin system (RAS). It is cleaved by renin to produce angiotensin I, which is further converted to angiotensin II, a potent vasoconstrictor that regulates blood pressure, fluid balance, and electrolyte homeostasis. AGT is primarily expressed in the liver and is involved in cardiovascular and renal physiology.
Disease Associations
| Disease category | Pathophysiological mechanism | Genomic evidence |
|---|---|---|
| Hypertension | Increased AGT expression or mutations lead to elevated angiotensin II, causing vasoconstriction and increased blood pressure. | ClinVar, OMIM |
| Preeclampsia | AGT variants contribute to placental ischemia and maternal hypertension during pregnancy. | OMIM, NCBI |
| Renal tubular dysgenesis | Loss-of-function mutations in AGT impair renal development, leading to neonatal renal failure. | OMIM, ClinVar |
| Chronic kidney disease | AGT polymorphisms are associated with progression of renal fibrosis and hypertension. | NCBI, OMIM |

Mutation functional classification
Loss of Function (LOF)
Mutations such as p.Met1Val cause complete loss of angiotensinogen production, leading to renal tubular dysgenesis.
Gain of Function (GOF)
Missense variants like p.Thr286Met increase angiotensin II production, contributing to hypertension.
Dominant Negative (DN)
No dominant-negative mutations reported for AGT.

Protein Summary
Angiotensinogen is a 485-amino acid glycoprotein primarily synthesized in the liver. It is the sole substrate for renin, initiating the RAS cascade. The protein contains a serpin domain but lacks inhibitory activity. Its cleavage by renin yields angiotensin I, which is further processed to angiotensin II, a key hormone in cardiovascular and renal regulation.
